Finally, the mainsteam media is starting to talk about patient-centered medical homes. These are a new delivery system aimed at coordinating and improving care delivery for people with chronic illnesses. Based in primary care providers' offices, these "medical homes" are team-based, with the patient at the center a full partner in health care delivery.
Read the NY Times article and think about how much better you would feel if you knew all of your doctors were talking to each other, working together, and making sure you are fully informed and involved. I love it. Jennifer
