A week or so ago, I posted Advocacy for Patients' comments on how insurers should calculate the medical loss ratio -- the portion of every premium dollar that is spent on health care. We argued that insurers should have to report separately the amount spent on utilization review and appeals -- the amount they spend trying to deny coverage to patients.
